What is the SD502 Form?
The SD502 is the official form used by NHS employees to opt out of the NHS Pension Scheme. It is administered by NHS Business Services Authority (NHSBSA) and must be submitted through your employer's payroll department.
The form requires both member details and employer verification. Once processed, you will no longer receive employer pension contributions or scheme benefits.
⚠️ Important: Your employer currently contributes 14.3% of your salary toward your pension. Opting out means losing this contribution and your guaranteed pension for life.
